I hereby declare that I accept the basic principles of Spiritualism, and apply to become a member of the Canberra Spiritualist Association Incorporated. If admitted as a member, I agree to be bound by the Rules of the Association and Code of Conduct for the time being in force.
Annual membership fee is $20 due in July each year.
